How to make an appointment

Initial appointments at any of our locations can be made by you, your referring physician, or your primary care physician.

Please call your preferred location for an appointment. Limited services are available at the Lexington and Waltham sites.

Please have the following information available when you call:

• Insurance information including insurance type (HMO, PPO, POS, etc.), group and/or member ID number, and subscriber name
• Your home, work and cell phone numbers, and email address

For referrals
Have your referring physician mail or fax a referral request (doctor's orders) with your child's name, date of birth, diagnosis, and reason for referral.

Call us to schedule your appointment at least 48 hours after the referral request has been submitted.
