Comprehensive Care

New Patients

Families of new patients can be relieved to know that the CAPE program will take care of your referral.  Contact Michelle Hernon at cape.program@childrens.harvard.edu or 617-919-3685.  She will take your child's personal information and schedule a consult to integrate your family in the CAPE program.

Existing Patients

Existing patients who haven't maintained constant contact with the CAPE team can also contact Michelle to schedule an appointment.  Lauren Perlman, our Respiratory Therapist, will then be in touch to gather further details.

Hospital Inpatients

If your child finds him or herself in Boston Children's Hospital they can still be seen by the members of the CAPE team.  Call Michelle for more information.

David Casavant, MD

Lauren Perlman, RT

Lauren Perlman is a registered respiratory therapist who cares for children with acute and chronic mechanical respiratory support needs.  Ms. Perlman trains patients, families and other healthcare providers to use long-term respiratory equipment, including ventilators, CPAP and BiPAP, suctioning, tracheostomies, cough assist and monitoring symptoms.  All care is coordinated with a CAPE program physician.

Julie Shurtleff, RN, MA, CPNP

Julie is a Nurse Practitioner who collaborates with the rest of the team including physicians, respiratory therapists and social workers to provide care for the patient. She shares in the responsibilities and care of the patient, which includes performing home visits, writing prescriptions, creating management plans and coordinating  care with multi-disciplinary teams and providers.

Yudy Muneton-Castano LCSW

Yudy is a licensed clinical social worker. Yudy meets with families and patients individually and completes psychosocial  and needs assessment, address child protection concerns, provide crisis interventions, consultation and advocacy,  provide assistance with concrete resources: financial, respite, Personal Care Attendants (PCA), long term care, utilities, housing, transportation, immigration etc. She also provides assistance with guardianship process, provide emotional support; coordinate and attend team/family meetings, coordinate care with outside agencies involved.

Michelle Hernon

Michelle serves as the introductory contact for the CAPE Program. She is available to help CAPE Program families with scheduling follow up appointments in the program and throughout the hospital, insurance referrals, medication refills and Durable Medical Equipment orders, or communicating with members of the CAPE Team.
