What is Medical Home Care Coordination?
Our practice uses evidence-based care and follows the guidelines and recommendations provided by the American Academy of Pediatrics (AAP).
Medical Home Care Coordination is part of a healthcare quality improvement mission.
Medical Home is not a place, nor does it refer to going to patients' homes, but rather it is an approach to providing comprehensive primary care that facilitates a partnership between patients, physicians, and families.
A Medical Home Care Coordinator can assist with the following:
- arranging appointments with specialists
- connecting families with community resources
- providing educational materials
