Your Visit | Overview

How do I make an appointment?

All patients must complete an intake questionnaire. Please visit the contact us page for additional details.

What needs to happen before the appointment?

Please contact your primary care physician or referring physician (i.e., otolaryngologist, neurologist, etc.) for an updated referral.

Once your intake paperwork has been reviewed and approved for scheduling, it may be necessary to obtain prior authorization from your insurance carrier. Please contact your insurance company for details. Our administrative team can assist with appropriate current procedural terminology (CPT) codes that may be needed for medical billing.

A patient whose family does not have their insurance referral and/or wishes to be seen outside their plan may pay for their visit at the time of service.

What should I bring to the appointment?

If your child uses hearing assistive technology (HAT), it is advised that they bring their HAT to the DHHP appointments. This may include hearing aid(s), cochlear implant(s), BAHA, and/or personal FM system.

We ask for any adult attending a visit at Boston Children’s please have a photo ID on them. Adults may be asked to show photo ID prior to entering the hospital.
