Insurance card
Due to the variety of insurances, with their individual requirements of co-payments and referral networks, we must insist that you keep a current valid insurance card with you when you come to the office for your visits. Inaccurate or out of date insurance information prevents us from receiving payment for the services we have rendered to you, therefore we will ask for your insurance card at each visit.
Co-payments
Please be prepared to pay your co-payment at the time of your visit. These conditions make it necessary for us to have proof of valid insurance before your child can be seen. We accept cash, checks, and Visa and Mastercard credit cards.
In the event that no proof of current insurability is available, we must ask that payment for services be made in full at the time of your visit. We ask for your understanding and indulgence in helping us with this issue, and we appreciate your cooperation.
