We at Newton-Wellesley Family Pediatrics feel the concept of the Patient Centered Medical Home (PCMH) has evolved into, what can now be considered, the future of pediatric primary care.
We would like to make it known that we have partnered with our colleagues at the Pediatric Physician’s Organization of Children’s Hospital (PPOC) to incorporate the concept of the Patient Centered Medical Home into our everyday practice.
Patient Centered Medical Home at Newton-Wellesley Family Pediatrics:

Our Medical Home team is comprised of two physicians, one Medical Home Care Coordinator/Nurse Manager, and our Practice Manager. Together they help balance our medical care with a strong supportive care program. PCMH and Newton-Wellesley Family Pediatrics puts you at the center of your care, working with your health care team to create a personalized plan for reaching your goals!
Medical Home Care Coordinator
Medical Home is an important part of Newton-Wellesley Family Pediatrics.
Cynthia Welch, RN is our dedicated Medical Home Coordinator. Her role is to work in partnership with the care teams at NWFP, our Practice Manager and the children and families in the practice to ensure that both the medical and non-medical needs of our patients are met.
She will function as an advocate for patient and family needs and will work collaboratively with our nursing and physician medical teams to serve as a resource and help establish access to services bridging the gap between NWFP and specialists, schools, and community connections (i.e. early intervention, family support, insurance coverage).

Complex Care Coordination
Complex Care Coordination is managed on an as needed basis by our Medical Home Care team.
Factors that are reviewed and indicate complex care needs are flagged for further management by our Clinical Nurse Manager (Cyndi Welch) and our Case Management Registered Nurses (Dee Douglas and Kingsley Richards). Resources will be discussed and offered along with coordination of care between practices.
Our coordinators will assist with obtaining and aligning community resource support as it pertains to the child. All case management occurrences will be documented for reference.
Behavioral Health
As part of the Newton-Wellesley Family Pediatrics commitment to comprehensive family-centered care, we offer service to Alexis Altman, LICSW: a licensed Independent Clinical Social Worker who is currently providing in-house psychological services for patients of Newton-Wellesley Family Pediatrics, in Newton, MA. It is an exciting position, addressing the integrated needs for patients with medical and mental health needs.
The practice is a member of the Pediatric Physicians Organization at Children's Hospital (PPOC) which is working closely with pediatricians to provide integrated medical and behavioral health services for their patients. Alexis is a close member of the pediatric care team and works closely with the Medical Home initiative. She is available for telephone and on-site mental health consultation, assessment, triage, and referrals. She is available on a limited basis for the treatment of children and families who are currently established with a NWFP PCP.
