Many insurance companies require a referral from your primary care physician before you receive treatment.

If your insurer requires a referral, please request it immediately so that it can be completed before your appointment.

5-day referral period

If your insurance company requires you to obtain a referral and you do not have one when you come for your appointment, you will be asked to sign a waiver stating that you acknowledge that you do not have a referral for the service that day and that you are financially responsible for payment of that service.

If you provide an insurance referral within five business days after the appointment, the waiver will be destroyed and your insurance will be billed.
